The School Health Law requires medical examinations for elementary school children on entry into school, and dental examinations in kindergarten and third grade. We are recommending that these examinations be done by your family physician and dentist since they can best evaluate your child's health and assist you in obtaining necessary treatments and corrections. The appropriate forms have been sent home.
Please return these forms on or before October 31st or call the nurse's office with the scheduled physical or dental date. If the forms are not received, then your child will be scheduled to see the school physician or dentist. If you need assistance obtaining medical insurance for your children, please contact the school nurse for available resources.
Color Vision Test: Grs. 1/2 (and previously untested)
Stereo Depth Perception Test: Gr. 1/2 (and previously untested)
Hearing Screening
K, Grades 1, 2, 3, 7 & 11
Scoliosis Screening
Grades 6 & 7
Body Mass Index (BMI) and BMI percentile
Yearly in grades Kindergarten through 12th
*All Screening exams may be rated as pass or fail. Failure means that the results are outside the expected range but are not a diagnosis of ill health. Parents are notified of all failures with regard to health screenings and an examination by an appropriate health care provider is recommended.
